FREE BASKETBALL TRAINING & SHOT EVALUATION

RELEASE OF LIABILITY AND VIDEO CONSENT FORM
In consideration of being allowed to participate in any activities organized by Basketball Pursuit Institute, LLC, including but not limited to basketball training, camps, events, and programs conducted by Coach Luke, Luke Condill, or any other associated individuals or organizations, I, the undersigned, agree and acknowledge the following:

1. Risk of Injury:
I understand that participation in basketball training and related activities involves inherent risks, which may include serious injury, permanent disability, or even death. While rules, safety measures, and equipment can reduce these risks, they cannot eliminate the possibility of serious injury. I acknowledge that these risks exist regardless of the precautions taken.
2. Assumption of Risk:
For Adult Participants: As an adult participant, I voluntarily assume all risks associated with my participation, whether known or unknown, even if those risks arise from the negligence of the Releasees (as defined below).
For Parents/Guardians of Minor Participants: As the parent/guardian of a minor participant, I voluntarily assume all risks on behalf of my child, whether known or unknown, even if those risks arise from the negligence of the Releasees (as defined below).
3. Monitoring Participation:
I agree that I will stop participation immediately if I observe any unsafe condition or concern regarding my (or my child’s) ability to safely participate. I will inform a program administrator or coach of any such concern as soon as possible.
4. Release of Liability:
I, the participant (or the parent/guardian of a minor participant), on behalf of myself, my spouse, my child, and our heirs, assigns, and personal representatives, hereby release Basketball Pursuit Institute, LLC, Lucas Condill, Coach Luke, other participants, sponsors, event organizers, and any venue owners or lessors (collectively referred to as "Releasees") from any and all claims of injury, disability, death, or property damage, arising from my (or my child’s) participation in these activities, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
5. Video and Photo Consent:
I grant permission to Basketball Pursuit Institute, LLC, and its agents to photograph and/or video record me (or my child) during training sessions, camps, events, or other related activities. I understand that these images and recordings may be used for promotional purposes, including but not limited to social media, websites, advertisements, and other media outlets. I waive any right to compensation or claims related to these images or recordings and release Basketball Pursuit Institute, LLC, from any liability related to their use.
6. Indemnification:
I agree to indemnify and hold harmless all of the above-mentioned Releasees from any and all liabilities, including legal fees, resulting from any claims or incidents related to my (or my child’s) participation in these activities, even if caused by the negligence of the Releasees, to the fullest extent permitted by law.
